On March 2, 2013, M-26 was in his vehicle, stopped and intending to exist gas station located north of 123rd Street on South Cicero Avenue in Alsip. The front, left quarter of Claimant's vehicle was struck after two southbound vehicles collided with each other. Following this incident, M-26 sustained an injury to his back. In June 2014, more than 15 months following the accident, M-26 underwent a microdiscectomy. Claimant originally injured his back in 2009 and then re-injured it a work in Fall 2012.

Settlement was reached a few days before the matter was scheduled for arbitration.
