In or about January 2014, M-50 was admitted to the nursing home facility. While at the nursing home facility, M-50 developed multiple decubitis (pressure) ulcers, urosepsis, and severe dehydration. During all times relevant, agents and/or employees of the nursing home were responsible for, inter alia, preventing, reporting, managing, and treating the resident's medical conditions.

During his residency at the nursing home facility in 2014, M-50 was taken to St. Bernard Hospital & Health Care Center multiple times due to deterioration of his health. M-50 died on January 20, 2015.

M-50 was survived by his two adult children.
